Our client provides mental health counseling, housing services, and employment assistance to adults, children, and their families. With locations all throughout Northern California and an almost 50 year history, these programs offer an opportunity to make a profound impact on the lives of hundreds, even thousands, of individuals.Β This position is based out of their Santa Rosa location.


Responsibilities

  • Provides leadership and guidance for clients as assigned in the absence of the Clinical Director or designee.
  • Supports clients in all aspects of therapy, assists them in requesting and receiving services, and gives them the tools they need to achieve their objectives.
  • advocates on behalf of and helps clients and their families obtain and receive, when necessary, medical care, shelter, food, and clothes, as well as mental health therapy.
  • supports each client's unique service plan.
  • maintaining all client records, finishing needed paperwork and data entry per the program's requirements, and other things like progress logs, activity reports, and logs.Β Β 
  • meets with clients on a regular basis at our locations or out in the community, at workplaces, residences, boarding and care facilities, skilled nursing institutions, and other suitable locations.
  • With the client's permission, consults or routinely meets with the client's family, caregiver, or other important people.
  • teaches customers how to handle their money, maintain good personal hygiene, and be job-ready in an age-appropriate manner.
  • aids in determining a client's eligibility for Medi-Cal or other benefits and, as necessary, fights for the continuance of benefits.
  • For customers who use representative payee services, create financial management plans and approve necessary expenses.
  • provides a defined client group with comprehensive case management services by constructively interacting within the team.
  • gives individuals "on-the-spot" therapy that is both beneficial to them and in line with the program's philosophy.
  • Drives a personal car when conducting program business for the purposes of medication outreach, home visits, client transportation, community/county meetings/trainings, and MH court.
  • focuses on the clients' wellness, health, and safety.
  • timely completion of documentation as assigned.
  • according to the requirements of the program, offers rotating "on-call" coverage after hours.


Requirements

  • 2 years of experience providing behavioral health services as a Case Manager, school counselor, behavioral health tech, or other similar position.
  • No licensure is required for this role.

Schedule

  • Tuesday-Saturday, 8am-5pm.
