Job Summary

Core-VA Solutions is looking for a versatile and self-motivated Case Manager Assistant to join our team. In this role, you will provide essential remote support to our clients across various industries. The ideal candidate will excel in managing a diverse range of Case Management tasks, including but not limited to:

Responsibilities:

  • Organize and maintain case files, records, and case-related documents while ensuring compliance, confidentiality, and secure handling of sensitive information.
  • Schedule and coordinate meetings, appointments, referrals, follow-ups, and other client-related activities, ensuring timely access to resources and services.
  • Act as a liaison between clients, service providers, families, and community organizations to address concerns and facilitate communication.
  • Assist in developing, monitoring, and updating individualized service plans, tracking deadlines, renewals, and progress to keep the team informed.
  • Monitor compliance with Medicaid, state, and program-specific regulations for service delivery and documentation.
  • Collaborate with healthcare providers, therapists, and service coordinators to ensure clients receive holistic care and support.
  • Prepare and submit reports on client progress, outcomes, and service utilization to maintain compliance and transparency.
  • Support case managers in resolving care-related concerns and addressing barriers to accessing required services.
  • Provide feedback to improve workflows, service delivery processes, and overall client satisfaction.


Requirements
  • Proven experience as a Case Manager Assistant or in a similar administrative role
  • Can work during US Central Time hoursΒ 
  • Experienced in working with US-based clients
  • Strong organizational, time management, and communication skills.
  • Proficiency in using office software (e.g., MS Office).
  • Ability to work independently, manage priorities, and meet deadlines.
  • Adaptability and problem-solving abilities.
  • Commitment to confidentiality and ethical conduct.
